Make a photocopy of the back of your lock. The lock must not be attached to anything. Master Lock does not provide combinations for locks that are attached to any items, such as bikes, lockers and cabinets.
Print the Lost Combination Form. The form is available on the Master Lock website.
Fill out the Lost Combination Form. Make sure you include the Master Lock's 903 serial number on the form. Attach a photocopy of the lock to the form.
Take the form to a notary public. Most banks and libraries offer notary public services. Show the notary public your valid driver's license. Sign the form in front of the notary public. The notary will sign and stamp the form. You may have to pay the notary a fee.
Mail the original notarized form and the photocopy of the lock to Master Lock. The mailing address is on the Master Lock website. You should receive your combination within four to six weeks.
