Download the "Lost Combination Form." The form is located on the Master Lock website (masterlock.com).
Fill out the form. Take the form to a notary public. Many local libraries offer free notary services. Show the notary a valid form of identification such as your driver's license. Sign the form in the presence of the notary. The notary will sign, date and stamp the form.
Get a photocopy of your lock's serial number. The serial number is located on the back case of the lock. The lock must not be attached to anything such as a locker or cabinet. Make sure you write the serial number on the photocopy as well.
Mail the original notarized form and the photocopy of your combination lock to Master Lock. The mailing address is listed on the Master Lock website. The processing time is approximately four to six weeks. Once your request is processed, you'll receive the lock's combination in the mail.
